- Three emergency exams per year during normal office hours

Smiles Membership (Premier Plan)

$50/month per employee
Regular Price:
$750/year
This comprehensive plan offers enhanced dental care, including:
- Up to four simple teeth cleanings per year, as recommended by the doctor
- Two complete annual dental exams
- All x-rays required for annual exams
- Four fluoride treatments per year
- Five emergency exams per year during normal office hours
Members also receive a 30% discount on all other dental procedures.
Family Plans
Smiles Membership Basic Family Plan (Corporate)
Cover the subscriber, spouse, and any dependents under 26 for just $40/month.
Smiles Membership Premier Family Plan (Corporate)
Cover the subscriber, spouse, and any dependents under 26 for $125/month.

The only requirement is that enrollees must stay active in the plan for at least six months before coverage can be terminated.
